2024-2025 Enrollment Application
To maintain our licensure with the State of Minnesota, it is required that we have certain documents on hand. Please fill out these forms and submit them before your child's first day of preschool. We are not able to have your child attend until all of these forms are filled out correctly and returned to us. If you need paper copies, we are happy to provide those as well.
